Jason was delighted to be asked to chair a discussion on Collaborative Models of Care Transformation with this impressive panel at the MIT Sloan Designing for Health Conference in December 2018.
Care for people with complex health needs drive much of the high cost of care in under-resourced populations. Most of this care, for over 75 million beneficiaries, is paid for by state and federal governments through Medicaid and Medicare. Several states are innovating in myriad ways in order to transform how care is delivered to those who need it most, and private payers are innovating in new ways as well. However, these innovations tend to be localized, minimally documented and infrequently researched. Opportunities to support this transformation abound, and panelists will discussed ongoing efforts, barriers, and enablers.
